Self-Determination Pre-Enrollment Supports are designed to assist individuals and their families in navigating the initial steps required to transition from traditional Regional Center Services to the Self-Determination Program (SDP).
What to expect: This service provides you with essential information, guidance, and hands-on assistance to ensure a smooth and successful entry into the program.
This will be broken up into several different meetings to ensure a smooth transition to SDP. You will receive information on the topic of Self-Determination, development of the Person Centered Plan, initial budget development, and guidance to help you understand the role of an Independent Facilitator.
